Jeana Reyes is an experienced clinical leader with over 25 years of healthcare industry experience in acute care, nursing education, population health management, and value-based care delivery. Jeana’s current role is the Director, Clinical Transformation and Quality for Clinical Reengineering, Value-Based programs at Horizon Blue Cross Blue Shield of New Jersey. She serves as the key business owner for developing clinical strategies and quality program models that lead to improving quality outcomes for the 1.9 million members attributed to Horizon’s Value-Based programs. She oversees and leads a cross-functional team that shares responsibility for provider relations, population health, quality data exchange, HEDIS and Stars performance within a matrixed organizational environment.   Jeana has a passion for adult learning theories which has led to the development of multiple clinical training programs grounded in adult learning theory principles that foster adult’s eagerness to connect with experience and motivation for self-improvement. She has served as an adjunct nursing faculty for an accredited nursing program at a local state college. Jeana is a member of the American Nurses Association, American College of Healthcare Executives, and American Association of Critical Care Nurses where she served on numerous committees over the past 15 years including Standards of Practice Task Force and National Certification Item Writing Committees. Jeana received her Master’s in Nursing with a post graduate certificate in Nursing Administration with a minor in Nursing Education from Thomas Edison University and is nationally board-certified in critical care nursing, cardiac surgery nursing and cardiac medicine nursing.
